  • Subject: “Whistleblowing” + brief summary.
  • Optional: request anonymity and a case ID for updates.

What happens next

  1. Intake and acknowledgment (or anonymized case ID if requested).
  2. Conflict check and assignment to an independent reviewer.
  3. Fact-finding with confidentiality and need-to-know controls.
  4. Outcome note and remediation actions where substantiated.
